Consent and Release of Liability

GENERAL CONSENT: I acknowledge that I am familiar with the general purpose of The Women Ministry Retreat.

ASSUMPTION OF RISK: I hereby voluntarily and knowingly assume all risks and dangers inherent and incidental participation in the Women Ministry Retreat, including special activities and transportation, understanding that some activities may pose a risk of injury.

RELEASE OF LIABILTY: I hereby release and hold harmless SPMBC its employees, agents, trustees, and volunteers from any and all actions, causes of action, suits, claims, obligations, liabilities, debts, demands, contentions, damages, judgments, levies, and executions of any kind, past, present, or future by reason of, arising out of, related to, or resulting from my participation.

PHOTOGRAPHY, DIGITAL, VIDEO RELEASE: I hereby grant SPMBC permission to use, without compensation or restriction, photographs, digital images and videotape images in I may appear, in any manner whatsoever such as, but not limited to: publication on SPMBC’s website or social media pages.

I understand that it is my responsibility to notify SPMBC if any information changes or I decide to withdraw my consent.

I attest that I have read, understand and consent to all terms outlined in this Registration & Consent Form
